Southend on Sea Adult Social Care Department work hard to continuously improve, and part of this journey has seen the creation and launch of our strategy and delivery model to support our social care provision called ‘Connected Southend’.
Our Connected Southend model provides the foundation for everything we do within social work, care provision, commissioning, back-office support functions, and much wider across our culture and arts offer.
Connected Southend is driven by the ambition to provide the very best care, support, and advice for the people of our city, and to truly embed the first line of the Care Act statutory guidance; “The core purpose of adult care and support is to help people to achieve the outcomes that matter to them in their life.”
Our Connected Southend model has been co-produced with people who use our services, local groups and partner organisations, to frame and shape all elements of our social care provision. Connected Southend also influences the work that our partners deliver in terms of integration and developing good practice.
The model itself sets out standards, principles and approaches for our staff and places more emphasis on prevention and enablement to support people to live the life they want.
In essence it is our new approach to providing the very best information, advice, prevention, support and social care. Alongside this Connected Southend has the heart of the city embedded within, through our use and support of voluntary and community groups, our arts, culture, leisure and our heritage.
Our vision is to work together with people to enable them to live safe, well, and independently across our communities, connected to the people and things they love and to live the life that’s important to them.
