Returning home from a stay in hospital can be overwhelming. Knowing there is extra support available can be just what is needed; helping patients to feel more confident about returning home.
The service offers friendly, informal help when patients are discharged. Having someone to lend a hand with shopping, collect prescriptions, visit for a cup of tea and a chat, or just to be there when people begin to do things for themselves, can help people regain their independence after a hospital stay.
There is also a handyperson service that patients can be referred to. This helps with things like moving furniture, installing key safes, fitting grab rails and keeping the home safe.
To get this service, patients in hospital can contact the discharge team directly. If patients return home and then decide they could use some help, they can contact us.
