Social care services for children and young people with disabilities in Southend are provided by the Children with Disabilities Team for 0 to 18 year olds, and by Adults and Community Services for those aged 18 years and over.

They provide support to children and young people (and their families) with disabilities such as:

• hearing loss
• visual impairment
• significant communication difficulties
• a severe learning disability or a moderate learning disability where there is an associated syndrome
• a physical disability
• a chronic or life threatening illness i.e. a degenerative or terminal condition.

Targeted support can be requested without referring to social services by any service or setting that a family already has contact with, such as a school, nursery or pre-school, or Health Visitor, by completing an Early Help Family Support Assessment.
