We're embarking on a significant change to improve the care you receive, and we want to hear from you. We're working with local health and care organisations to launch a new Shared Care Record for the Mid and South Essex Integrated Care System. This is more than just an IT update – it's a step towards better, more coordinated care for everyone in our community.
The Shared Care Record will bring together important information from health and care records held by different services into one place. It will mean that whether you're visiting your GP, in hospital, or receiving social care, the professionals caring for you will have access to a more complete, up-to-date picture of your health and care history. This could make all the difference in how quickly and effectively you're treated.
To get this right, we need to understand your views. Your experiences, concerns, and ideas are vital in shaping how the Shared Care Record works for you and the people you care about. That's why we're inviting you to take part in a short survey. It's your chance to influence how your health and care information is managed and shared.
The survey is open until Sunday 14 January 2024. It won't take much of your time, but your input could have a lasting impact. You can find the survey online at https://virtualviews.midandsouthessex.ics.nhs.uk/connecting-your-care. If you require a paper copy of this survey, please contact us at mse.sharedcarerecord@nhs.net.
We also understand that you might have questions or need more information before you feel comfortable sharing your views. That's completely okay. Alongside the survey, we're here to answer questions you might have about the Shared Care Record.
This isn't just about a new computer system – it's about making sure your care is as good as it can be, by ensuring that your health and care team have the information they need. By sharing your thoughts, you're helping us move towards a future where health and care services in mid and south Essex are more connected, efficient, and tailored to your needs.
Please, take a moment to help us shape this important development. Your voice matters.
