Service overview

The team provide support to children and young people aged 0 to 18, and their families, with disabilities such as:

  • significant hearing loss, visual impairment or communication difficulties
  • severe learning disability or a moderate learning disability where there is an associated syndrome
  • severe physical disability
  • chronic or life threatening illness (i.e. a degenerative or terminal condition).

Targeted support can be accessed by completing an Early Help and Family Support Assessment with:

  • a health professional
  • school SENCO
  • person responsible for additional needs at college
