How do I complete a referral?

You can complete this referral for yourself, or on behalf of another person.

At the start of the referral, you will see questions relating to your everyday activities. Only select those that relate to you.

Depending on your answer you may be asked to provide more detail using a free text field or by answering additional questions.

You do not need to answer every question, just what is relevant. Any questions that must be answered will be marked with a (*).

Based on your responses given within your referral, and once submitted, a member of our team will contact you.

Portal Online Accounts

Introduction

A Portal Online Account allows you to complete forms online.

Who can have an account?

You can hold the account, or someone else can make a referral on your behalf with your consent.

Information contained in your account

Your account will contain the following information:

  • your basic details, including address, telephone numbers, date of birth
  • any forms that you complete within the account

These documents will contain sensitive information about:

  • your support needs
  • your health
  • your history and family situation
  • any services or support that you receive

All this information will be visible to you or anyone acting on your behalf.

What you need to set up an account

  • an email address

Registering an account

You will be able to register an account via the Portal. You can do this either by selecting the "Register for an account link," or when submitting a form for the first time.

Setting up a Portal Online Account

This guidance will help you complete each step to register for a Portal Online account.

You'll need an email address. This is because you'll use it every time you log in.

How to register for an account

From the Secure login page select the "Register for a new account" button.

You will then be asked to enter your name, address, email address and to create a password. Fields marked with a * are mandatory and must be completed to be able to create your account.

Please note that your password must meet the following requirements:

  • at least 12 characters long
  • contain at least one letter
  • contain only letters, digits, and special characters
  • must start with a letter
  • contain at least one upper-case letter
  • contain at least one numerical digit
  • contain at least one special character
  • be different to your current password
  • must be different to your previous 8 passwords

After registering via the Portal, you'll receive an email from Southend-on-Sea Adult Portal titled 'Please verify your email address."

If you have not yet received it, check your spam folder.

The email will contain instructions on how to proceed, including a unique 8-digit code, which you will need to make a note of.

Return to the sign in page and put the code into the 'Verification Code' field, then select 'Finish'.

How to log in to your account after registration

  1. go to the Southend Adult Social Care Portal page
  2. use the top menu to select 'My Account' to go to the secure login screen
  3. at the "login" screen, enter your email address and password within the Existing Users section, then select the 'Submit' button:
  4. without closing the sign in page, go to your email inbox. You'll have received an email titled "Your custom code for Southend-on-Sea City Council Adult Social Care Portal login'. The email will contain an 8-digit code. Note that this email could take a few minutes to arrive. If you can't see it, check your spam folder
  5. return to the sign in page and put the code into the 'Code' field, then select 'Finish'
  6. you'll be able to view your account

How to log out of your account

The information contained within these accounts is sensitive, so it's important to remember to log out of your account whenever:

  • you have finished using it
  • you're leaving your device unattended.

You can log out by selecting your name at the top of the screen and then choosing 'Logout' from the dropdown list.

The system will automatically log out after 15 minutes of inactivity. It will display a 5-minute countdown after 10 minutes of inactivity.

If you start typing or navigating, the timeout will disappear. If the period of inactivity continues to 15 minutes, it will log out.

Forgotten Password

If you have forgotten your password, you can reset it using the "Forgotten Password" link within the Secure Login screen.

At the next screen, enter your email address and select 'Next'. This will take you to the "Reset Password" screen.

  1. Go to your email account. You'll have received a message from Southend-on-Sea Adult Portal with the subject line 'Please verify your email address'. Open this to locate the 8-digit verification code.
  2. Go back to the reset password screen, enter the code in the 'Code' field and select "Next."
  3. At the next screen you need to enter your new password twice, once in each box, then select 'Finish'.

Please note that your password must meet the following requirements:

  • at least 12 characters long
  • contain at least one letter
  • contain only letters, digits, and special characters
  • must start with a letter
  • contain at least one upper-case letter
  • contain at least one numerical digit
  • contain at least one special character
  • be different to your current password
  • must be different to your previous 8 passwords
  1. You'll then go back to the 'Sign in' page. You have successfully changed your password.
  2. Now you can enter your email address and new password and continue with the rest of the login process.

Using a Portal Online account

This guidance will help you with using and updating your Portal Online account once you are logged in. You can access these options by selecting your name at the top of the page.

Recover a Form

This option contains forms that you have started, but not completed. You can view them at any time to update them or print/download them to your device:

  1. Select the option of "Recover a form" from the drop-down menu.
  2. Select the name of the form you wish to view.
  3. Once you have opened the form you can edit it or save it to your device or print it by selecting the icons above or below it. These may vary depending on the browser or device you are using.

The form will remain within the "Recover a form" section of your account, until it is submitted. Please note that the form will be available for 30 days. If not submitted within that period, the form will be deleted.

Submitted Forms

This option contains forms that you have submitted. You can view them at any time and can print/download them to your device:

  1. Select the option of "Submitted Forms" from the drop-down menu.
  2. Select the name of the form you wish to view.
  3. Once you have opened the document you can save it to your device or print it by selecting the icons above or below it. These may vary depending on the browser or device you are using.

Update Details

Within the Update Details screen you can:

  • view your account details.
  • change your profile details.
  • change your password.

Account Details

The Account Details section of the page shows the email address linked to the account. The "View More" button provides you with your unique ID.

Change Profile Details

You can change your name, address, telephone number or email address by visiting your Portal account.

Changes made to your details here, won't change other systems within Southend City Council.

The details of your account will display, and you will be able to change them, as necessary. Once amended then select the 'Finish' button at the bottom of the page.

If you change your name, you'll see the change within the welcome message when you login to your Portal account.

Changing your password

It is recommended that you change your password on a regular basis to ensure that your account remains secure.

Within the Change Password screen, you will be asked to enter your current password and then your new password.

Please note that your password must meet the following requirements:

  • at least 12 characters long
  • contain at least one letter
  • contain only letters, digits, and special characters
  • must start with a letter
  • contain at least one upper-case letter
  • contain at least one numerical digit
  • contain at least one special character
  • be different to your current password
  • must be different to your previous 8 passwords

Once you have entered your new password, select the "Finish" button.
